Japanese encephalitis is spread by Culex mosquitoes that breed in flooded rice paddies and around pig farms. That single fact explains most of the advice for Thailand. The risk sits in the rural, agricultural countryside, not in the centre of Bangkok or on a resort beach.
If your trip is a week or two in Bangkok, Phuket, Koh Samui or the other islands, your risk is low and the vaccine is usually unnecessary. If you are spending a month or more upcountry, working or volunteering on farms, cycling or trekking through rural areas, or staying near rice-growing land during the wet season, the vaccine is advised. A full course is two doses of Ixiaro, the only Japanese encephalitis vaccine licensed in the UK. On the standard schedule the doses are 28 days apart, and protection begins seven days after the second dose. Adults aged 18 to 65 who are short of time can use an accelerated schedule with the two doses just seven days apart. Children under 18 and adults over 65 use the standard spacing.
For Thailand, that means ideally booking four to five weeks before you fly, though the accelerated option gives adults a faster route if departure is close.
